Admissions
Admissions Process
  • Each applicant will be considered on the basis of his/her individual needs.

  • No individual will be denied services based on race, color, national origin, sex, or age.

  • Must be 18 years of age or older.

  • Must show proof of disability.

  • QC C.A.R.E Residential Services serves primarily those with a diagnosis of:

+ Mental Retardation
+ Developmental Disabilities
+ Brain Injury

  • Work Services serves those listed above and people needing vocational rehabilitative services.

  • Applicants must have third party sponsorship for funding or be able to pay for services.

  • Referrals normally come from County Central Point of Coordination Administrator (CPC) or Vocational Rehabilitation Services; may come from self, families or other interested persons.

 
Application & Admissions
  • Application information will be reviewed by QC C.A.R.E's Admissions Committee. The ability of QC C.A.R.E to serve the individual will be determined. A meeting with the individual to gather more information may be requested.

  • The applicant will be notified of approval or denial for waiting list. If approved, individual's name will be added to the waiting list under the licensure that best meets his/her needs. The individual will be considered for vacancies when they arise or when services can begin.

  • The approved applicant should notify his/her county of legal settlement to inform them of his/her intentions for services at QC C.A.R.E.

  • When accepted for services, a current psychological evaluation and physical examination (within the past year) will be required prior to admission for Residential Services.

  • When a vacancy is accepted by the applicant, an admission date will be set. Necessary approvals for licensure's and funding will be secured. A packet of information will be sent for completion and a Pre-Admission meeting will be scheduled 1-2 weeks prior to the move. An Admission meeting will be held on the admission date. Staff will assist with transition.
